Personal Outcomes Measures

In line with the organisation's commitment to change and development Wisdom Services has adopted a Personal Outcome Measures (POMs) quality model to reflect our vision, mission and values.

POMs is an internationally recognised quality system. It was developed in the US by the Council on Quality and Leadership for use with people with a disability. The system is used extensively in Ireland.

The model is used to ask our service users what they want and how they would like to be supported. Using the measures we focus on the choices people have in their lives. Priorities to improve the individual’s quality of life are clarified. A Person Centred Plan is drawn up which clearly identifies personal goals and the supports required to achieve them.

Wisdom Services view POMs as an effective tool for ongoing continuous evaluation, influencing how we as an organisation individualise supports to facilitate a measurable outcome for our service users.
