Clinical Nurse Specialist - Dementia Care
Wisdom Services is currently one of the few within the intellectual disability field to have a well established, comprehensive Dementia Care programme.
This service provides dementia assessment and full diagnostic screening conducted by a Clinical Nurse Specialist in Dementia Care who works in conjunction with our medical/psychiatry/psychology services. Dementia specific programme planning is individually tailored and monitored by this experienced multi-disciplinary team.
The service works in partnership with other specialities in sourcing interventions to promote the best possible health outcomes for persons with dementia. An Ageing-in-Place model is advocated and supported throughout the continuum of dementia. For those persons with more advanced dementia a high-support nursing unit is available supported by speech and language therapy, social work, chaplaincy and others.
The social networks of each person with dementia are identified. The role of these networks and particularly the paramount role of families is greatly valued, nurtured and supported. A well resourced library is available on site and education and training programmes have been developed however the individual living with dementia continues to be our greatest teacher in the development of a person-centred service.
